Salary: From £14.00 to £15.77 per hour

Job description

Post will be based at the Brierley Hill Health and Social Care Centre working in the Community but will also involve home working too. Will need to be able to drive and have access to their own vehicle. Applicant will be required to work 5 days full time across 7 days

* To be involved in the Care Management process under the overall responsibility of a Senior Social Worker or Assistant Team Manager.

* To work with vulnerable adults to assess their needs and contribute to the development of support plans that enable people to achieve the optimum level of independence.

* To provide a sensitive, responsive and appropriate response to people who use services and their Carers. To Be involved in the process of Care Management under the overall responsibility of the Senior Care Co-ordinator/Team Leader. To work to provide a sensitive, responsive and appropriate response to individual service users and their Carers. Qualifications: 5 GCSE’s or equivalent including Maths and English It is essential for the post holder to have experience of the users group to which the post relates and also some understanding of the assessment process and the issuing of equipment. 1 post available
